How this is calculated
The albumin-globulin (A/G) ratio compares two classes of proteins found in blood serum. Albumin is the most abundant plasma protein — made by the liver — and serves as a carrier for hormones, fatty acids, and drugs while also maintaining osmotic pressure. Globulins are a heterogeneous group that includes clotting factors, enzymes, antibodies (immunoglobulins), and transport proteins like haptoglobin and transferrin.
Because a standard comprehensive metabolic panel (CMP) reports both total protein and albumin but not globulin separately, globulin must be derived: Globulin = Total Protein − Albumin. The A/G ratio then shows which fraction dominates. A normal ratio is roughly 1.0–2.0 (some labs use 1.1–2.1); values vary slightly by laboratory and assay. A low A/G ratio (< 1.0) may indicate reduced albumin production (liver disease, malnutrition) or elevated globulins (chronic infection, autoimmune disease, multiple myeloma). A high A/G ratio (> 2.0) is less common and can reflect low globulin production.
