How this is calculated

The Absolute Neutrophil Count (ANC) quantifies the number of mature neutrophils — the body's primary bacterial defence — in a cubic millilitre of blood. It is derived from the CBC differential by multiplying the total white blood cell count (WBC, in K/μL) by the fraction of segmented (mature, "segs") neutrophils, converted to cells/μL: ANC = WBC × 1,000 × segs% ÷ 100, which simplifies to WBC × segs% × 10.

The "without bands" distinction matters clinically. The full ANC formula includes both segmented neutrophils and immature band cells: ANC = WBC × (segs% + bands%)/100. Excluding bands — as automated analysers increasingly do — gives a slightly lower and more conservative ANC. Some oncology protocols and infection risk assessments specify "ANC without bands" to avoid inflating the count with immature cells that have reduced functional capacity.

Clinical thresholds follow the Common Terminology Criteria for Adverse Events (CTCAE v5): ANC ≥ 1,500 cells/μL is normal; 1,000–1,499 is Grade 1 (mild) neutropenia; 500–999 is Grade 2 (moderate) neutropenia; below 500 is Grade 3/4 (severe) neutropenia, carrying a significant risk of life-threatening infection (febrile neutropenia). Frequently asked questions

ANC with bands includes both segmented (mature) neutrophils and band (immature/stab) neutrophils in the percentage: ANC = WBC × (segs% + bands%). ANC without bands excludes the band count and uses only segs%. Modern automated haematology analysers may not reliably differentiate bands from segs, so "without bands" has become a common reporting standard, especially in oncology.

An ANC below 500 cells/μL (Grade 3/4 neutropenia) is considered severe. Patients with ANC < 100 cells/μL are at extreme risk for overwhelming bacterial and fungal infections and typically require hospitalization, protective isolation, and broad-spectrum antimicrobial therapy. An ANC below 1,500 cells/μL (any neutropenia) warrants monitoring even if the patient feels well.
