How this is calculated

The Barthel Index, developed by Mahoney and Barthel in 1965, scores ten activities of daily living (ADLs): feeding, bathing, grooming, dressing, bowel control, bladder control, toilet use, transfer (bed to chair), mobility on level surfaces, and stair use. Each activity receives a weighted score reflecting its importance to independent living. Feeding, dressing, bowel and bladder control, toilet use and stair climbing are each scored 0/5/10; bathing and grooming are scored 0/5; transfer and mobility are scored 0/5/10/15.

The maximum score of 100 indicates complete functional independence. Cut-off scores used clinically include: 0–20 = total dependence; 21–60 = severe dependence; 61–90 = moderate dependence; 91–99 = slight dependence; 100 = independent. A change of at least 5 points is generally considered clinically meaningful. The scale measures actual performance, not capability — score the best activity the patient reliably achieves, not ideal or occasional performance.

Limitations: the BI does not capture social functioning, cognition, pain or quality of life. It was validated primarily for neurological and orthopaedic rehabilitation patients and may be less sensitive at the extremes of function. Always use alongside clinical judgment.

Frequently asked questions

Scores of 85–100 are associated with the ability to live independently, while scores above 60 generally indicate the potential for community living with support. There is no universal discharge threshold — clinical teams use the BI alongside home environment and support network assessments.

The original Barthel Index uses three score levels per item. The modified Barthel Index (Shah et al., 1989) uses a five-level scale for each item to increase sensitivity to change during rehabilitation. This calculator uses the original Mahoney–Barthel scoring.

Yes — the BI assesses functional performance regardless of cognitive status, so it is used in dementia care to track physical independence. However, it does not measure cognitive decline or behavioural symptoms, for which separate tools (such as the MMSE or BIMS) are required.
