Each task is rated 0 (unable) to 4 (independent performance), giving a maximum of 56 points. The scale was designed for elderly populations but is now widely used across neurological and musculoskeletal rehabilitation.

The total score maps to three clinical fall-risk categories: 0–20 suggests the patient is wheelchair-dependent and at high risk of falling; 21–40 indicates the patient can walk with some assistance and is at moderate risk; 41–56 suggests independent ambulation and low fall risk. A score drop of 4 points or more often indicates a clinically meaningful change, while the minimal detectable change (MDC₉₅) is approximately 5 points — changes smaller than this may fall within measurement error.

Limitations: the BBS was validated on community-dwelling elderly adults; it may show ceiling effects in higher-functioning individuals and floor effects in very impaired patients.
