How this is calculated
The Bruce Protocol is a standardised graded exercise treadmill test originally developed by Robert A. Bruce in 1963 for diagnosing coronary artery disease. It uses seven 3-minute stages of progressively increasing speed and incline: Stage 1 starts at 1.7 mph and 10% grade; each subsequent stage increases both speed and gradient, reaching 6.0 mph at 22% in Stage 7. The total exercise time strongly correlates with maximal aerobic capacity (VO₂max), making it a practical indirect estimate without direct gas analysis.
The regression equation for males (Bruce et al., 1973) is VO₂max = 14.8 − 1.379T + 0.451T² − 0.012T³, where T is total exercise time in minutes. For females, the McConnell & Clark (1987) equation VO₂max = 4.38T − 3.9 is commonly used. These equations were derived from populations of adults tested to volitional exhaustion. The calculator also shows the stage VO₂ from the ACSM metabolic equations (which use the actual treadmill speed and grade) as a cross-reference — the regression-based estimate from total time is the standard Bruce Protocol output.
VO₂max is divided by 3.5 mL/kg/min to express the result in METs (metabolic equivalents), where 1 MET = resting oxygen consumption. The fitness categories shown use ACSM reference ranges for adults. Note that the Bruce Protocol regression equations are valid for healthy adults — they may overestimate VO₂max in cardiac patients or those with significant deconditioning, and are not validated for children. Always use clinical exercise testing with physician supervision in populations with known or suspected heart disease.

VO₂max is the maximum rate at which the body can consume oxygen during intense exercise, expressed in mL of O₂ per kg of body weight per minute. It is the gold standard measure of cardiorespiratory fitness. A higher VO₂max means the cardiovascular and muscular systems can deliver and use more oxygen, enabling greater sustained exercise intensity.
The Bruce regression equations have a standard error of estimate of about ±3.5 mL/kg/min (±1 MET), meaning the result could realistically be 1–2 METs above or below the true value. Direct measurement with metabolic gas analysis is more accurate. The indirect estimate is useful for population-level fitness screening but should not be used alone for individual clinical decision-making.
The Bruce Protocol requires a motorised treadmill with programmable speed and incline settings. For home use, a standard treadmill capable of reaching 6+ mph and 22% incline is needed. For individuals with heart disease, hypertension, or other cardiovascular risk factors, it should be performed under physician supervision with ECG monitoring. The Modified Bruce Protocol starts with lower-intensity warm-up stages for less fit or older individuals.
