How does this calculator work?
Max safe dose (mg) = min(body weight × mg/kg limit, absolute ceiling). Divide by (concentration% × 10) to get the maximum volume in mL. How this is calculated
Local anesthetics block sodium channels in nerve membranes, preventing action-potential propagation. At doses above a weight-based threshold they can cause systemic toxicity — initially CNS symptoms (perioral tingling, tinnitus, metallic taste, dizziness) and, in severe cases, seizures or cardiovascular collapse. The maximum dose is therefore expressed as mg per kg of body weight, with an absolute ceiling that applies regardless of patient size.
The calculator multiplies the patient's weight by the agent-specific mg/kg limit, then caps the result at the absolute maximum for that drug. Dividing this dose by the solution concentration converts milligrams to millilitres: a 1% solution contains 10 mg/mL, 0.5% = 5 mg/mL, 2% = 20 mg/mL, and so on. If you enter a planned dose, a safety-band meter shows how close it is to the maximum.

Frequently asked questions
Epinephrine causes local vasoconstriction, slowing the absorption of lidocaine into the systemic circulation. This prolongs the local effect and reduces the peak plasma concentration, allowing a higher total dose before toxic levels are reached — 7 mg/kg versus 4.5 mg/kg for plain lidocaine.
Early CNS signs include perioral tingling, metallic taste, tinnitus, dizziness and visual disturbances, progressing to confusion and seizures. Cardiovascular toxicity — arrhythmias and cardiac arrest — can follow, especially with bupivacaine. Treatment is intravenous lipid emulsion (Intralipid); facilities using local anesthetics should have a lipid rescue protocol available.
A 1% solution contains 1 g (1,000 mg) per 100 mL = 10 mg/mL. Multiply the percentage by 10: 0.25% = 2.5 mg/mL, 0.5% = 5 mg/mL, 1% = 10 mg/mL, 2% = 20 mg/mL.
