How this is calculated

The gestational sac is the first structure visible on transvaginal ultrasound, typically appearing at around 4.5–5 weeks. Because the sac is roughly spherical but not perfectly so, three orthogonal internal diameters are measured and averaged to give the mean sac diameter. This reduces measurement error from any single plane.

The gestational age estimate uses the empirical formula GA (days) = MSD (mm) + 30, derived from Hellman et al. (1969) and widely cited in obstetric imaging guidelines. For example, an MSD of 20 mm corresponds to approximately 50 days (7 weeks 1 day). The formula is most accurate between 5 and 8 weeks; beyond 8 weeks, crown–rump length (CRL) is the preferred dating method.

Note that MSD-based dating carries a typical margin of ±1 week and can differ from dates based on the last menstrual period (LMP) or CRL. Empty sac (no embryo visible when MSD ≥ 25 mm), or a sac too small for dates, are clinical findings that require physician evaluation — this calculator is for educational estimation only.

Frequently asked questions

The gestational sac is irregularly shaped and collapses slightly under transducer pressure. Averaging three orthogonal diameters gives a more reproducible estimate than any single measurement and reduces plane-specific distortion.

MSD is most useful from about 4.5 to 8 weeks. Before 4.5 weeks the sac may not be seen; after 8 weeks the crown–rump length of the visible embryo provides more accurate dating.

An MSD ≥ 25 mm without a visible embryo on transvaginal ultrasound is a criterion for a failed pregnancy (empty sac / anembryonic pregnancy). A second scan several days later is often performed before a diagnosis is made. Always consult your healthcare provider.
