How this is calculated

The MIPI (Mantle Cell Lymphoma International Prognostic Index) was developed by Hoster et al. (Haematologica, 2008) from a multivariate analysis of 455 MCL patients treated in the German Low Grade Lymphoma Study Group trials. The four independent predictors are: patient age (linear, continuous), ECOG performance status (binary — 0 if PS ≤ 1, 0.6978 if PS ≥ 2), the ratio of serum LDH to the laboratory upper limit of normal on a log10 scale, and the peripheral leukocyte count in /μL on a log10 scale.

Cut-off scores divide patients into three risk groups: low risk (MIPI < 5.7, ≈ 60% 5-year overall survival in the derivation cohort), intermediate risk (5.7 ≤ MIPI < 6.2, ≈ 35% OS), and high risk (MIPI ≥ 6.2, ≈ 20% OS). These survival figures are historical estimates from patients treated in 1998–2006 and may not reflect outcomes with modern targeted therapies such as BTK inhibitors or CAR-T cells.

WBC is entered in cells per μL (e.g. 6 000 for 6 × 10³/μL). If your lab reports in 10⁹/L, multiply by 1 000. The LDH ULN is lab-specific — enter your institution's stated upper limit (commonly 240 U/L). Results must be interpreted by a qualified haematologist in the context of full clinical, pathological and imaging findings.
