The phrase "3 weeks pregnant" causes confusion because it depends on the reference point. In obstetric convention, gestational age is counted from the first day of the last menstrual period (LMP). "3 weeks gestational" therefore means days 14–20 from the LMP — which is actually the week of ovulation, when conception has just occurred or has not yet happened. hCG does not begin rising until the fertilised egg implants, typically 6–12 days after ovulation, so a test taken at 3 weeks gestational (LMP-based) will almost always be negative.
Most people who say "I am 3 weeks pregnant" actually mean they are 3 weeks past a missed period, which corresponds to roughly 5–6 weeks gestational (LMP-based) and around 35 DPO. At this stage hCG has been rising for approximately 3–4 weeks and is typically in the hundreds to thousands of mIU/mL — a very clear positive on any standard test.
