How does this calculator work?
BODE = BMI score (0–1) + FEV₁% score (0–3) + mMRC dyspnea score (0–3) + 6-minute walk score (0–3); range 0–10. Higher scores indicate worse COPD prognosis. Approximate 4-year survival: Q1 (0–2) ~80%, Q2 (3–4) ~67%, Q3 (5–6) ~57%, Q4 (7–10) ~18% (Celli et al. 2004). For clinical use by qualified practitioners only.

How this is calculated
The BODE index was introduced by Celli et al. in 2004 (NEJM 350:1005–1012) to capture the systemic nature of COPD beyond airflow limitation alone. It combines four independently scored components to produce a total from 0 (least severe) to 10 (most severe).
The B component scores body-mass index: a BMI at or below 21 kg/m² scores 1 point (underweight indicates worse nutritional status and outcomes), above 21 scores 0. The O component scores post-bronchodilator FEV₁ as a percentage of the predicted normal: ≥65% = 0, 50–64% = 1, 36–49% = 2, ≤35% = 3. The D component uses the modified Medical Research Council dyspnea scale (0–4): mMRC 0–1 = 0, mMRC 2 = 1, mMRC 3 = 2, mMRC 4 = 3. The E component scores the 6-minute walk test distance: ≥350 m = 0, 250–349 m = 1, 150–249 m = 2, ≤149 m = 3.
In the original 2004 validation cohort, approximate 4-year survival by quartile was: Q1 (0–2) ~80%, Q2 (3–4) ~67%, Q3 (5–6) ~57%, Q4 (7–10) ~18%. These are population-level estimates; individual prognosis depends on exacerbation history, comorbidities, treatment and many other factors. The BODE index is a clinical research and communication tool, not a diagnostic criterion.
