How this is calculated

The Glasgow Coma Scale (GCS) assesses consciousness along three independent dimensions: eye opening (E, 1–4), verbal response (V, 1–5), and motor response (M, 1–6). Each subscore reflects the best response the patient demonstrates — the scale measures capacity, not the worst observed behaviour. The three values are summed to give a total between 3 and 15.

Severity is classified by the total: 3–8 = severe brain injury (coma); 9–12 = moderate brain injury; 13–15 = mild or normal. A GCS of 8 or below is the widely used airway-protection threshold, though the definitive clinical decision depends on trajectory and aetiology. Reporting individual subscores alongside the total (e.g. E4 V5 M6 = GCS 15) gives more information than the total alone.

The scale has limitations. Intubation prevents verbal scoring (recorded as 'T'); periorbital swelling prevents eye scoring (recorded as 'C'). In those cases the affected subscore is omitted from the sum and documented separately. GCS was validated primarily in traumatic brain injury and performs somewhat differently in metabolic or hypoxic coma. It is a bedside screening tool, not a diagnostic test.

A GCS of 15 (E4 V5 M6) indicates a fully alert and oriented person: eyes open spontaneously, answers questions coherently, and follows commands. It is the best possible score and is normal for a healthy, awake adult.

A GCS of 8 or below is the commonly cited threshold at which a patient is generally unable to protect their own airway. Most guidelines recommend considering airway management at or below this score, though clinical judgment, the cause, and the trajectory of the score all inform the decision.

If a patient is intubated, the verbal response cannot be assessed. Report the eye and motor subscores and append 'T' for the verbal component (e.g. E3 VT M5 = 8T). Some centres impute V1 in this situation; always document the method used.
