Different opioids vary widely in potency. Morphine Milligram Equivalents (MME) create a standardised "common currency" so clinicians can compare doses across agents. The CDC 2022 Clinical Practice Guideline publishes conversion factors — MME per milligram of drug — which this calculator applies: MME/day = daily dose × factor.
For oral morphine the factor is 1.0 by definition; oxycodone is 1.5×; hydromorphone 4.0×; and tramadol only 0.1×. Fentanyl transdermal patches are measured in mcg per hour, not mg per day; the CDC factor of 2.4 MME per mcg/h converts patch strength directly (a 25 mcg/h patch gives 25 × 2.4 = 60 MME/day). Methadone is unique: its potency increases with dose because of tissue accumulation, so the CDC applies a step-wise factor (4 MME/mg for ≤ 20 mg/day, rising to 12 MME/mg for > 60 mg/day).
The risk meter reflects CDC 2022 thresholds: doses above 50 MME/day warrant increased clinical vigilance; above 90 MME/day the CDC recommends careful reassessment of risks and benefits.
