How this is calculated
Glucose Infusion Rate (GIR) quantifies the rate at which glucose (dextrose) is delivered to a patient, expressed in milligrams per kilogram of body weight per minute. It is the primary metric used in neonatal and paediatric intensive care to assess and titrate glucose support, and to avoid both hypoglycaemia and hyperglycaemia.
The derivation of the formula: a D10W (10% dextrose) solution contains 10 g per 100 mL = 100 mg/mL. At a rate of R mL/hr, the delivery rate is R × 100 mg/hr ÷ 60 min/hr = R×100/60 mg/min. Generalising to any concentration D%: delivery = D × 10 × R / 60 = D × R / 6 mg/min. Dividing by body weight W gives GIR = (D × R) / (6 × W) mg/kg/min.
For term neonates, a GIR of 4–8 mg/kg/min is generally considered the physiological target for glucose homeostasis; premature infants often need 6–8 mg/kg/min. A GIR below 4 mg/kg/min may be insufficient, while above 10–12 mg/kg/min risks hyperglycaemia and hepatic steatosis. For term neonates, the target GIR is generally 4–8 mg/kg/min. Premature or very-low-birth-weight infants often require 6–8 mg/kg/min. Rates above 10–12 mg/kg/min can cause hyperglycaemia and should be used with caution or with insulin support.
Higher concentrations (e.g. D12.5–D15) deliver more glucose per mL and are often used via central venous access to limit fluid volume. Peripheral lines generally tolerate up to D10–D12.5. Adjust concentration and rate together to achieve the target GIR within safe fluid and osmolarity limits for the access site.
The 6 factor converts units: D% × 10 mg/mL × rate mL/hr ÷ 60 min/hr = D × rate / 6, giving mg/min, which is then divided by weight in kg to give mg/kg/min. Using 6 avoids two separate conversion steps and is the standard clinical shorthand.
