How this is calculated
The Charlson Comorbidity Index, published by Charlson et al. in 1987, assigns integer weights (1, 2, 3 or 6) to 17 major comorbid conditions based on how strongly each is associated with 1-year mortality in hospitalised patients. The base score is the sum of those weights across all conditions the patient has. An age adjustment adds one point for each decade of age over 40 (age 40–49 = +1, 50–59 = +2, 60–69 = +3, and so on), producing the age-adjusted CCI.
The 10-year survival estimate is derived from the regression equation published in the original 1987 paper: P = 0.983^exp(CCI × 0.9). At a score of 0 this gives 98.3% survival; each additional point substantially lowers survival because of the exponential relationship. The categories used here (0 = low, 1–2 = mild, 3–4 = moderate, ≥5 = high burden) reflect common clinical interpretation of the index.
The CCI was developed in a specific hospital cohort and the survival formula applies most reliably to in-patient populations in a similar era and setting. Subsequent validation studies (Deyo 1992, Quan 2011) updated the condition list and weights slightly, and survival rates in modern populations differ from 1987 due to advances in treatment. Use the index as a research and risk-stratification tool, not as a clinical prognosis tool for individual patients.

The original 1987 index covers 17 conditions grouped by weight. Weight-1: MI, CHF, peripheral vascular disease, cerebrovascular disease, dementia, COPD, connective tissue disease, peptic ulcer, mild liver disease, uncomplicated diabetes. Weight-2: hemiplegia, moderate-severe renal disease, diabetes with end-organ damage, any non-skin malignancy, leukemia, lymphoma. Weight-3: moderate-severe liver disease. Weight-6: metastatic solid tumor, AIDS/HIV.
One point is added to the CCI for each decade of age above 40. Age 40–49 adds 1 point, 50–59 adds 2, 60–69 adds 3, 70–79 adds 4, 80–89 adds 5, and so on. This reflects that older patients have poorer outcomes for any given comorbidity burden.
The survival formula from Charlson 1987 provides a population-level estimate, not an individual prediction. Modern survival rates are higher than those from 1987 due to medical advances. The estimate is useful for comparing risk between groups in research, not for counselling individual patients.
