How this is calculated

Neutrophils are the front-line immune cells that destroy bacteria and fungi. The Absolute Neutrophil Count quantifies how many are circulating per microlitre of blood. It is derived from two numbers on a complete blood count (CBC) with differential: the total white blood cell count (WBC) and the percentage of the WBC that are neutrophils — both segmented (mature, "segs" or "polys") and banded (slightly immature, "bands").

The standard formula multiplies the WBC expressed in thousands per microlitre by the combined neutrophil percentage and then by 10 to convert to cells per microlitre: ANC = WBC × (segs% + bands%) × 10. For example a WBC of 5.5 ×10³/µL with 60% segs and 5% bands gives ANC = 5.5 × 65 × 10 = 3,575 cells/µL, which is normal.

Clinical thresholds: ANC ≥ 1,500 cells/µL is normal; 1,000–1,499 is mild neutropenia; 500–999 is moderate neutropenia with meaningful infection risk; below 500 (profound or severe neutropenia) carries a high risk of serious bacterial and fungal infections and is the threshold at which prophylactic antibiotics or growth factors (G-CSF) are often considered in chemotherapy protocols. Values are population references and clinical decisions depend on the clinical context, trend, and patient history.

Normal ANC in adults is generally 1,500–8,000 cells/µL. Values below 1,500 indicate neutropenia: mild (1,000–1,499), moderate (500–999), or severe (<500). Severe neutropenia significantly raises the risk of life-threatening bacterial and fungal infections.

Band neutrophils are slightly immature neutrophils released from bone marrow during increased demand (a "left shift"). They are functionally similar to mature segmented neutrophils, so both are included in the ANC. Some automated CBC analysers report bands separately; if your report does not show bands, enter 0.

ANC is routinely monitored in patients receiving chemotherapy, immunosuppressive therapy, or biological agents that can suppress bone marrow. It is also measured to evaluate chronic neutropenia, aplastic anaemia, or infections where a very low count may alter antibiotic and isolation decisions.
