MME Calculator — Morphine Milligram Equivalents
Convert an opioid dose to morphine milligram equivalents (MME/day) using CDC 2022 clinical conversion factors — helps compare opioid regimens and assess dose against CDC risk thresholds.

此計算機如何運作?
MME/day = daily opioid dose × CDC 2022 factor. Key factors: morphine 1.0, oxycodone 1.5, hydromorphone 4.0, tramadol 0.1, fentanyl patch 2.4 per mcg/h, methadone 4–12 (dose-dependent). CDC caution thresholds: 50 MME/day (increased risk) and 90 MME/day (high risk). All dosing decisions must be confirmed with a clinician.

How this is calculated
Different opioids vary widely in potency. Morphine Milligram Equivalents (MME) create a standardised "common currency" so clinicians can compare doses across agents. The CDC 2022 Clinical Practice Guideline publishes conversion factors — MME per milligram of drug — which this calculator applies: MME/day = daily dose × factor.
For oral morphine the factor is 1.0 by definition; oxycodone is 1.5×; hydromorphone 4.0×; and tramadol only 0.1×. Fentanyl transdermal patches are measured in mcg per hour, not mg per day; the CDC factor of 2.4 MME per mcg/h converts patch strength directly (a 25 mcg/h patch gives 25 × 2.4 = 60 MME/day). Methadone is unique: its potency increases with dose because of tissue accumulation, so the CDC applies a step-wise factor (4 MME/mg for ≤ 20 mg/day, rising to 12 MME/mg for > 60 mg/day).
The risk meter reflects CDC 2022 thresholds: doses above 50 MME/day warrant increased clinical vigilance; above 90 MME/day the CDC recommends careful reassessment of risks and benefits. These are population-level benchmarks, not absolute limits — individual clinical factors always apply. This calculator is an educational reference; all dosing decisions must be confirmed with a qualified prescriber.
常見問題
A Morphine Milligram Equivalent (MME) expresses any opioid's analgesic potency relative to oral morphine. It lets clinicians compare doses across drugs — for example, 10 mg oxycodone (1.5 MME/mg) equals 15 MME, equivalent to 15 mg oral morphine.
Methadone accumulates in body tissues with repeated dosing and its effective half-life lengthens with dose. A low methadone dose is roughly 4× as potent as morphine by weight, but high doses can reach 12×. The CDC step-wise table accounts for this unusual pharmacokinetics.
No — they are clinical approximations based on population-level studies, intended for prescribing oversight rather than precise equianalgesic substitution. Genetic variation, tolerance, renal function, and drug interactions mean actual doses must be adjusted individually.
