ACR (mg/g) = urine albumin (mg/dL) × 1,000 ÷ urine creatinine (mg/dL). Values below 30 mg/g are normal (A1), 30–300 mg/g indicate moderate albuminuria (A2), and above 300 mg/g indicate severe albuminuria (A3), each associated with increasing chronic kidney disease and cardiovascular risk.

How this is calculated

Healthy kidneys retain albumin in the bloodstream; leakage into urine signals kidney damage. Because urine concentration varies with hydration, dividing albumin by creatinine (which is filtered at a nearly constant rate) corrects for dilution. The result, the albumin-to-creatinine ratio (ACR), is expressed in milligrams of albumin per gram of creatinine (mg/g) and is reproducible across random spot samples without the inconvenience of a 24-hour collection.

When both albumin and creatinine are measured in mg/dL, creatinine in g/dL equals creatinine in mg/dL divided by 1,000. Dividing albumin (mg/dL) by that value gives ACR in mg/g — which is algebraically equivalent to multiplying by 1,000 before dividing. The calculator uses this exact arithmetic.

KDIGO (2012) classifies albuminuria into three categories: A1 (ACR < 30 mg/g) is normal to mildly increased; A2 (30–300 mg/g) is moderately increased (formerly "microalbuminuria"), linked to early CKD and cardiovascular risk; A3 (> 300 mg/g) is severely increased (formerly "macroalbuminuria"), indicating more advanced kidney involvement. A single elevated ACR should be confirmed with two further tests over three months before a diagnosis is made.

An ACR below 30 mg/g is considered normal to mildly increased (category A1). Values between 30 and 300 mg/g indicate moderate albuminuria (A2), and above 300 mg/g indicates severe albuminuria (A3). Your doctor will interpret the result in the context of your full clinical picture.

A spot urine ACR is as predictive as a 24-hour urine albumin in most clinical settings and far easier to obtain. Dividing albumin by creatinine corrects for variable urine dilution, making the result reliable regardless of hydration status.

Strenuous exercise, urinary tract infections, fever, heart failure, and very high blood pressure can transiently raise urinary albumin. A borderline or elevated result should be confirmed on two further samples over at least three months.
